Forms for Joining the Troop

Below you can download all the forms you need to register as a  Scout and/or Adult Leader with the Boy Scouts of America and Troop 323.  

Scouts

Scouts must fill out a BSA Application Form and Troop 323 Application Form for Scouts.  

On the BSA Application Form for Scouts, all we need completed is Page 5 ("Boy Scout Application - Local Council Copy") - skip Pages 6 & 7 - we use the standalone Class 1 medical form instead of Page 7.

Prior to your first outing, the Permission Slip, Class 1 and Class 3 Medical Forms must all be completed.  The Class 1 Medical Form should be filled out by the Scout's parent or guardian and must be notarized.  The Class 3 Medical Form requires a physical examination by a doctor and must be signed by the doctor.  These 3 forms must all be renewed annually.

Adults

A parent or guardian must fill out Troop 323 Registration Form for adults.  Any adult wishing to volunteer as an Adult Leader or drive Scouts on outings must also complete a BSA Application Form for adults, plus Class 1 and Class 3 Medical Forms.

On the BSA Application Form for Adults, all we need completed is Page 3 ("Adult Application - Local Council Copy") - skip Pages 4, 5 & 6.

Forms for Re-Registering with the Troop

Once a year the Troop must re-charter with the Boy Scouts of America, and every Scout and Adult Leader must re-register and pay their dues.

Below, you can find the current forms required to be completed upon re-registration.

Notes:  

All Scouts and Adult Leaders participating in Troop events are required to have Class 1 medical forms on record with the Troop.

All Scouts and Adult Leaders participating in any event involving strenuous activity such as backpacking, high altitude, extreme weather conditions, cold water, exposure, fatigue, athletic competition, adventure challenge, or remote conditions where readily available medical care cannot be assured require a Class 3 medical form on record with the Troop. Examples include high-adventure activities, jamborees, Wood Badge, Summer Camps and overnight backpacking trips in remote areas.  Medical information required includes current health history supported by a medical evaluation within the past 12 months performed by a licensed health-care practitioner.

Both Class 1 and Class 3 Medical Forms must be updated annually and put on record by the Troop Membership Chair.  Class 1 Medical Forms are filled out by the Scout's parent or guardian and must be notarized.  Class 3 Medical Forms require a physical examination by a doctor and must be signed by the doctor.
